We are dedicated to compassionately providing patient-centered care. Our goal is to help patients and their loved ones achieve the highest quality of life possible by providing physical, emotional, and spiritual care. We are experienced in managing the signs and symptoms caused by the advanced stages of the disease process. Our team will provide care to the patient while remaining in the comfort of their home instead of making numerous visits to the hospital.
Our highly trained team consists of Physicians, Therapists, Nurses, Certified Nursing Assistants, Social Workers, Chaplains, and Volunteers.
Our Physicians are responsible for the overall medical component of our program, and they are the medical resource for the interdisciplinary team. They are available to the nurses 24/7 for questions, concerns, or changes in the patient's condition.
Our nurses are responsible for direct patient care, which includes patient and caregiver education. A registered nurse will perform ongoing nursing assessments, evaluate medications, and monitor changes in the patient's condition. Typically the skilled nurse will visit one to two times a week, and the hospice aide will visit two to three times a week or more often as the patient's condition changes.
Our psychosocial team consists of social workers and chaplains. They are responsible for assessing the patient and family's emotional and spiritual needs. They will help you prioritize what is important to you and your family. For example, they can help you navigate power of attorney, living wills, DNRs, and funeral arrangements.
They will provide emotional and spiritual support while assisting with life-changing decisions. Most importantly, your beliefs and values will always be respected.
Our bereavement team specializes in family care and counseling. They may provide support and education to help you and your family members, loved ones, and caregivers understand their grief journey. Katy Hospice, LLC offers group support as well as individual support. We actively reach out to your loved ones for 13 months but are available for as long as needed.
